I, {name}, the undersigned parent or legal guardian, acknowledge and agree to the following:

I give permission for my child to participate in youth fitness classes offered by Small Gym Big Fitness. I understand that participation in physical activity involves inherent risks, including but not limited to falls, collisions, sprains, and other potential injuries.

I hereby voluntarily assume all risks associated with my child’s participation. I release and discharge Small Gym Big Fitness, its owners, employees, volunteers, and representatives, as well as the property owners of 1807 Lakeview Drive, from any and all liability, claims, demands, or causes of action that may arise from any injury, illness, accident, or loss incurred during or resulting from participation in these classes or presence on the premises.

I certify that my child is physically able to participate and that I have disclosed any relevant medical conditions to staff. I also agree to hold harmless Small Gym Big Fitness and the property owners of 1807 Lakeview Drive and waive any right to legal action against them and their affiliates.
